Occupational Therapist (OT) – Home Health

Job Summary
Origin Home Health Care is seeking a compassionate, dependable, and patient-focused Occupational Therapist (OT) to join our growing home health team. The Occupational Therapist is responsible for evaluating patients and developing individualized treatment plans to improve independence, safety, and quality of life within the home setting.
The ideal candidate is motivated, organized, and passionate about helping patients regain and maintain functional abilities through skilled therapeutic interventions.
Responsibilities
Perform occupational therapy evaluations and reassessments in accordance with physician orders and agency policies
Develop and implement individualized treatment plans based on patient needs and goals
Provide therapeutic interventions to improve activities of daily living (ADLs), mobility, strength, coordination, and safety awareness
Educate patients, families, and caregivers on home exercise programs, adaptive equipment, and fall prevention strategies
Assess home environments and recommend modifications to maximize patient independence and safety
Collaborate with physicians, nurses, physical therapists, speech therapists, social workers, and other interdisciplinary team members
Complete all clinical documentation accurately and within required timeframes
Participate in case conferences and care coordination meetings as needed
Maintain compliance with Medicare, state, federal, and agency regulations
Monitor patient progress and adjust treatment plans as appropriate
Qualifications
Current and active Occupational Therapist license in the State of Florida
Graduate of an accredited Occupational Therapy program
Minimum of one (1) year of experience preferred; home health experience strongly preferred but not mandatory.
Current CPR certification
Valid driver’s license, reliable transportation, and current auto insurance
Strong clinical assessment and communication skills
Ability to work independently and manage time effectively
Knowledge of Medicare documentation requirements preferred

Requirements
Job Requirements
Current and active Florida Occupational Therapist (OT) license in good standing
Graduate of an accredited Occupational Therapy program
Minimum one (1) year of clinical experience preferred
Home health experience preferred but not required
Current CPR certification
Valid driver’s license, reliable transportation, and active auto insurance
Ability to travel to patient homes within assigned coverage areas
Strong assessment, clinical decision-making, and problem-solving skills
Excellent communication and interpersonal skills
Ability to work independently and manage time efficiently
Knowledge of Medicare, OASIS, and home health documentation standards preferred
Ability to complete documentation accurately and within required deadlines
Comfortable using electronic medical records (EMR) systems and mobile documentation platforms
Ability to establish and maintain effective working relationships with patients, families, physicians, and interdisciplinary team members
Demonstrates professionalism, compassion, and patient-centered care at all times
Ability to safely perform essential job functions, including standing, walking, bending, lifting, and assisting patients as needed
