Qualifications:
Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
Registered with the National Registration Examination of the American Occupational Therapy Association
Two years of clinical experience as an occupational therapist
Minimum of 1 year of Home Health experience preferred
Demonstrates good verbal and written communication, and organization skill
Possesses and maintains current CPR Certification
Valid drivers license

Competitive compensation based on experience, knowledge, skills, and location, as well as internal equity and alignment with market data
Estimated Annual Full Time Compensation: $153K to $206K (This position is paid on a per-visit basis plus mileage)

Requirements
Required Qualifications
Graduate of an accredited Occupational Therapy program approved by the Accreditation Council for Occupational Therapy Education (ACOTE) or equivalent accrediting body
Current Occupational Therapist (OT) license in the State of California
Current registration/certification through the National Board for Certification in Occupational Therapy (NBCOT)
Minimum of two (2) years of clinical experience as an Occupational Therapist
Minimum of one (1) year of Home Health experience preferred
Current CPR/BLS certification
Valid California driver’s license, reliable transportation, and current auto insurance
Ability and willingness to travel throughout assigned service areas including Hayward, San Leandro, Alameda, and Oakland
Clinical Responsibilities & Skills
Evaluate and assess patients’ functional status, mobility, cognitive abilities, and activities of daily living (ADLs)
Develop and implement individualized treatment plans based on physician orders and patient goals
Provide therapeutic interventions to improve independence, safety, and quality of life in the home environment
Recommend adaptive equipment, durable medical equipment (DME), and home modifications as appropriate
Educate patients, families, and caregivers on treatment plans, safety strategies, and therapeutic techniques
Collaborate effectively with physicians, nurses, physical therapists, speech therapists, social workers, and interdisciplinary care teams
Accurately complete documentation in a timely manner in compliance with agency, state, federal, and Medicare guidelines
Monitor patient progress and modify treatment plans as needed
Maintain patient confidentiality and uphold professional and ethical standards of practice

Physical Requirements
Ability to travel to patient homes throughout assigned territories
Ability to lift, transfer, and assist patients as necessary
Ability to perform prolonged standing, walking, bending, reaching, and driving throughout the workday
Preferred Qualifications
Experience with Medicare home health documentation and regulatory standards
Familiarity with EMR/documentation systems used in home health settings
Bilingual or multilingual capabilities strongly preferred
