Company Profile

Senior Care Partners PACE

Company Overview

The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Company History

Comprehensive Senior Care Corporation (CSCC) was founded in 2006. Senior Care Partners
P.A.C.E. (SCP) is the first program to be developed through CSCC. SCP is the name of the PACE
programs in Calhoun, Kalamazoo, and Eastern Van Buren, Southeast Allegan, and Southern Barry
Counties. Start?up funding for the Calhoun County program was provided by the Battle Creek
Community Foundation and the Kellogg Foundation. The first provider application for a PACE
Program was submitted to the Centers for Medicare and Medicaid Services (CMS) in May 2008.
The first participants were enrolled in Calhoun County on April 1, 2009. Through guarantor
partnerships with Battle Creek Community Foundation, Senior Services of Southwest Michigan,
Bronson Healthcare Group, and Hospice Care of Southwest Michigan a second Senior Care
Partners P.A.C.E. center was opened in February of 2013 to support Kalamazoo County (and later,
expanded services to support Eastern Van Buren County). The first participants were enrolled in
Kalamazoo County on March 1, 2013. As of September 1, 2018, participants from Southeast
Allegan and Southern Barry Counties were included in the geographic service area. On May 1st,
2020, the Albion Center opened and began serving participants in the greater Marshall and
