Essential Functions
Conduct thorough chart review and screening for appropriateness of patient for OT evaluation.
Perform initial evaluation including history of prior level of function, subjective information, objective tests and measures, assessment and establishment of plan of care and goals for future treatments. Perform home safety evaluations as needed. Perform reevaluation at required intervals and assess patient progress against goals.
Incorporate evidence based clinical decision making to select and utilize appropriate patient interventions in accordance with plan of care, and modify plan of care as appropriate based on patient progress/response to interventions. Perform reevaluation at required intervals and assess patient progress against goals.
Understand therapy billing, coding, and payor reimbursement. Document and bill accurately per compliance guidelines.
Identify patient’s adaptive equipment needs, instruct patients and caregivers in safe and correct use of equipment, and collaborate with other disciplines/equipment suppliers to ensure patient’s equipment needs are met.
Maintain clear documentation records and use clinical judgment to determine when additional communication beyond standard documentation is required for safe and coordinated patient care.
Maintain patient safety, modesty, and privacy/confidentiality at all times.
Provide education to patients, caregivers and other staff members as appropriate.
Provide supervision and guidance as needed to Certified Occupational Therapist Assistants and rehabilitation aides.
Collaborate and communicate effectively with other team members including physicians, case managers, nurses, and other therapy staff, including participation in discharge planning or care coordination.
Screen for and make referrals to other disciplines when indicated, including mandatory reporting in cases of suspected abuse/neglect.
Meet required licensure and continuing education requirements.
Required to maintain active Basic Life Support (CPR) certification.
Other duties as assigned.

Qualifications
Required:
Active Washington State Occupational Therapist License or Active Occupational limited permit with expectation to obtain Active Washington Occupational Therapist license in the first year.
Graduated from an accredited occupational therapy program Rehab Services Manager
